Effects of this medicine

This medicine works on central nervous system in order to control epilepsy seizures.
It is usually used to treat partial/generalized/mixed seizures.

Before using this medicine, be sure to tell your doctor and pharmacist

  • If you have previously experienced any allergic reactions (itch, rash, etc.) to any medicines.
  • If you are pregnant or breastfeeding.
  • If you are taking any other medicinal products. (Some medicines may interact to enhance or diminish medicinal effects. Beware of over-the-counter medicines and dietary supplements as well as other prescription medicines.)

Dosing schedule (How to take this medicine)

  • Your dosing schedule prescribed by your doctor is ((        to be written by a healthcare professional))
  • In general, for adults, take 100 to 200mg of the active ingredient in 1 to 3 divided doses on the first day. Then, increase the dose every 1 to 2 weeks to 200 to 400mg in 1 to 3 divided doses a day.
    For children, take 2 to 4mg/kg of the active ingredient in 1 to 3 divided doses on the first day. Then, increase the dose every 1 to 2 weeks to 4 to 8mg/kg in 1 to 3 divided doses a day.
    The maximum dose is 600mg of the active ingredient a day for adults, 12mg/kg of the active ingredient a day for children. 1 tablet contains 100mg of the active ingredient. Strictly follow the instructions of your doctor and pharmacist.
  • If you miss a dose, take the missed dose as soon as possible. However, if it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. You should never take two doses at one time.
  • If you accidentally take more than your prescribed dose, consult with your doctor or pharmacist.
  • Do not stop taking this medicine unless your doctor instructs you to do so.

Precautions while taking this medicine

  • This medicine may cause sleepiness, diminished attention, loss of concentration, decreased reflex motor capacity, etc. Therefore do not drive a car or operate dangerous machinery while taking it.

Possible adverse reactions to this medicine

The most commonly reported adverse reactions include sleepiness, loss of appetite, enervation/aspontaneity, motor incoordination (clumsy of mouth and incoordination of movements, light-headed feeling), nausea/vomiting, lassitude/feeling of weariness, decreased mental activity (enervation, indifference, loss of concentration), rash, itch and decreased sweating. If any of these symptoms occur, consult with your doctor or pharmacist.
The symptoms described below are rarely seen as initial symptoms of the adverse reactions indicated in brackets. If any of these symptoms occur, stop taking this medicine and see your doctor immediately.
  • fever, erythema/blister spreading across a wide area, skin exfoliation [toxic epidermal necrolysis, mucocutaneous ocular syndrome, dermatitis exfoliativa]
  • anemic condition, sore throat, generalized fatigability, bleeding tendency [aplastic anemia, agranulocytosis, pure red cell aplasia, thrombocytopenia]
  • fever, dry cough, breathing difficulty [interstitial pneumonia]
  • pain/stiffness/numbness in the muscles of the limbs [rhabdomyolysis]
  • lumbar backache, abdominal pain, bloody urine [nephrolithiasis, urolithiasis]
The above symptoms do not describe all the adverse reactions to this medicine. Consult with your doctor or pharmacist if you notice any symptoms of concern other than those listed above.

药物剂型

药理作用
本品为苯并异唑衍生物，能抑制癫痫病灶，阻滞癫痫发作放电的扩散速度。用于治疗对其他药物治疗无效的癫痫，特别是部分性发作。对原发性全身发作、混合型癫痫、肌阵挛性发作及癫痫综合征如伦-加综合征和韦斯特综合征有效。但通常用作其他第一线药物的辅助治疗。


药动学
本品口服易吸收，血药浓度达峰时间约为5〜6h，红细胞中药物浓度为血浆浓度的4〜9倍。tl/2为60h。经肝脏代谢，随尿排出。


适应证
用于各型癫痫的辅助治疗。


禁忌证
孕妇、哺乳者和对本品过敏者禁用。


注意事项
1.慎用：
(1)肝肾功能不全者慎用；
(2)驾车和机械操作者应慎用。
2.连续用药中不可急剧减量或突然停药。
3.用药期间应定期检查肝功能。


不良反应
1.常见不良反应有皮疹、倦怠、头痛、眩晕、烦躁、抑郁、幻觉、平衡障碍、食欲缺乏、恶心、呕吐、腹痛、胃痛、腹泻、白细胞减少、贫血、血小板减少及肝酶值升高。
2.少见的不良反应有视觉异常、体重减轻、发热、发汗、口炎和肾结石。


用法用量
1.成人开始口服每天100〜200mg，分1〜3次服，1〜2周可加量至每天200〜400mg，最大剂量每天600mg。
2.儿童开始每天2〜4mg/kg，分1〜3次服，1〜2周可加量至每天4〜8mg/kg，最大剂量每天12mg/kg。


药物相应作用
苯妥英钠、苯巴比妥、卡马西平和丙戊酸钠可降低唑尼沙胺的血药浓度。其他抑制肝CYP3A4的药物，也能降低唑尼沙胺的血药浓度。由于本品的不良反应可能与碳酸酐酶抑制作用有关，故尽量不与其他碳酸酐酶抑制剂(如乙酰唑胺、托吡酯)合用。 


临床研究
本品为广谱抗癫痫药物，临床上应用于各种类型的癫痫发作均有一定的疗效，也可用于治疗癫痫持续状态 。
